We are seeking a highly skilled and dedicated Registered Nurse to join our team as a Transition Line Specialist. In this role, you will have the opportunity to consolidate your skills and knowledge in a specialty nursing unit, while also contributing to the delivery of high-quality patient care.
Key Responsibilities- Assess patients and collect information from various sources to develop a comprehensive care plan.
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and effective communication.
- Provide direct patient care, including administering medications, performing procedures, and monitoring patient outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Stay up-to-date with the latest clinical practices and research, and apply this knowledge to improve patient care.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Completion of the applicable post-graduate specialty nursing course/program and/or willingness to complete the applicable post-graduate specialty nursing courses/program.
- Current certification in Basic Cardiac Life Support (BCLS).
