Oak Valley Health is seeking a skilled Social Worker to join our team. As a Social Worker, you will play a critical role in providing psychosocial support to patients and their families in the acute care setting.

About the Role:

  • Provide short-term, goal-oriented support and counseling to patients and families dealing with grief, chronic life-altering issues, and end-of-life concerns.
  • Work with the healthcare team, patient, and family to develop patient care plans.
  • Complete psychosocial assessments and maintain up-to-date knowledge of community resources.
  • Consult with community healthcare professionals and attend committees and task forces as required.
  • Cross-cover for other Social Workers within the hospital.

Requirements:

  • Master of Social Work degree.
  • Registration with the Ontario College of Certified Social Workers and Social Services Workers.
  • Recent related previous social work experience in a hospital environment.
  • Understanding of loss/adjustment and the grief process.
  • Demonstrated strengths in interpersonal relations, problem-solving, and conflict resolution.
  • Demonstrated excellent communication skills.
  • Demonstrated commitment to professional development.
  • Experience in working with electronic documentation, preferably Meditech.
  • Demonstrated good attendance and performance records with the ability to maintain these same standards.
