Job Title: Social Worker - Community Care Specialist

Job Summary:

We are seeking a highly skilled and compassionate Social Worker to join our team at Bayshore HealthCare. As a Social Worker - Community Care Specialist, you will work with clients and their families to provide support and guidance in navigating the healthcare system. You will assess clients' needs, develop and implement care plans, and provide education and resources to promote optimal health and well-being.

Key Responsibilities:

  • Assess clients' needs and develop individualized care plans
  • Provide education and resources to clients and their families
  • Collaborate with healthcare professionals to ensure comprehensive care
  • Monitor clients' progress and adjust care plans as needed
  • Document client interactions and care plans in a timely and accurate manner

Requirements:

  • Master's or Bachelor's degree in Social Work
  • Active membership in good standing with the Ontario College of Certified Social Workers and Social Service Workers
  • Professional liability insurance ($5 million minimum)
  • Police clearance that meets Bayshore HealthCare's policy
  • General computer skills and access to a computer with password protection and internet capabilities

Preferred Qualifications:

  • Minimum of two years of community-based therapy experience
  • Valid driver's license and access to a vehicle in good operating condition
  • Current CPR certification
