Seeking a compassionate and experienced Registered Nurse/Registered Psychiatric Nurse to provide person-centered care to individuals with mental health and substance use disorders in a community or acute setting.
Responsibilities- Coordinate care for clients by collaborating with interprofessional care teams, conducting mental health assessments, developing and implementing comprehensive treatment plans, and administering prescribed medications.
- Provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and implementing treatment modalities such as ADLs and wound care.
- Work from a strengths-based model, using motivational interviewing and engagement techniques to develop and implement individualized care plans.
- Establish and maintain therapeutic relationships with clients and their informal support networks, promoting empowerment and self-care.
- Provide treatment services, including crisis intervention, individual counseling, and education, using a variety of theoretical models.
- Initiate and participate in case conferences/meetings with clients, family, service providers, and members of the interprofessional team to ensure a coordinated approach to treatment.
- Consult and collaborate with members of the interprofessional care team to identify and resolve issues, adjust care plans as required, and ensure continuity of care.
- Monitor Extended Leave as required under the Mental Health Act, reviewing conditions and tracking renewal of certification due dates.
- Aid client access to services, including medical appointments, hospitals, and social programs, by providing healthcare information and assisting with processing forms and transportation issues.
- Document assessments, care plans, observations, interventions, client/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Maintain a safe environment for clients, visitors, and employees by adhering to established safety and emergency practices and procedures.
- Identify own learning needs/goals, review them with the Team Coordinator, and engage in activities to meet these needs/goals.
- Participate in research and quality improvement activities by identifying client care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures, or protocols.
- Graduation from an approved School of Nursing or Psychiatric Nursing.
- Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- One (1) year of recent, related experience providing treatment services to individuals with mental health and/or concurrent disorders in a community or acute setting.
- Valid Class 5 BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
