About the Role

We are seeking a Registered Practical Nurse (RPN) to join our team in Windsor and Essex. As an RPN with SE Health, you will have the opportunity to work in a supportive team environment, with flexible schedules and competitive pay.

Key Responsibilities
  • Complete comprehensive client assessments, develop care plans, and implement and evaluate care based on best evidence and practices.
  • Support clients through care transitions by collaborating and connecting them with community support services.
  • Work as a primary provider for a client caseload, accountable for their care while working in collaboration with other care team members.

Requirements
  • Current registration with the College of Nurses of Ontario.
  • Canadian Nurses Association (CNA) specialty certification an asset.
  • Enthusiasm and love of community care, with a passion for evidence-informed practice and excellence in care.
  • A current CPR certificate and valid driver's license.
  • Updated immunization records and a clear criminal background check.
