Job Summary: * Provide treatment and care management services to individuals with mental health and/or concurrent disorders * Collaborate with interprofessional care teams to develop and implement comprehensive treatment plans * Conduct mental health assessments, administer prescribed medications, and monitor client progress * Provide direct nursing care to clients, including reviewing client/family history, conducting physical/psychosocial assessments, and developing care plans * Establish and maintain therapeutic relationships with clients and their informal support networks * Provide education and support to clients and their families on managing mental health and substance use issues Responsibilities: * Coordinate care for clients by collaborating with interprofessional care teams and performing intake functions * Develop, implement, and maintain comprehensive treatment plans based on client needs and best practices * Provide direct nursing care to clients, including administering medications and monitoring client progress * Establish and maintain therapeutic relationships with clients and their informal support networks * Provide education and support to clients and their families on managing mental health and substance use issues * Participate in case conferences and meetings with clients, family, and service providers to ensure coordinated care * Consult and collaborate with members of the interprofessional care team to identify and resolve issues and adjust care plans as needed Qualifications: * Graduation from an approved School of Nursing or Psychiatric Nursing * Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM) * Two (2) years recent related experience providing treatment services to individuals with mental health and/or concurrent disorders * Valid Class 5 BC Driver's License and access to personal vehicle with appropriate insurance coverage Skills and Abilities: * Ability to complete initial and ongoing mental health and substance use client assessments and provide nursing care through appropriate technical, therapeutic, and safety interventions * Knowledge of mental illness and substance use treatment * Knowledge of psychopharmacology (indications and side-effects) * Counselling and group facilitation skills * Ability to communicate effectively verbally and in writing with clients, families/care givers, and members of the interprofessional care team * Ability to promote person-centered care that demonstrates empathy for clients/families/care givers, sensitivity to diverse cultures and preferences, and advocacy for special populations About Fraser Health: Fraser Health is a leading healthcare provider in Metro Vancouver and the Fraser Valley, serving over two million people.
