Join our team at SE Health and make a meaningful impact on the lives of seniors. As a Health Care Aide, you will be part of a diverse and collaborative care team that provides care and independence to residents in seniors living facilities.
About the Role:
- Provide personalized care and support to residents, including personal care, activities of daily living, and assistance with meal delivery.
- Assist with recreational activities and leisure activities as per individual care plans.
- Provide Medication Assistance Program requirements as per resident care plan.
- Report changes in resident conditions and any other pertinent information to the LPN in a timely manner.
- Demonstrate flexibility and adaptability in a fast-paced environment.
- Participate in the development and updating of care plans.
- Charting within approved documentation system daily.
- Actively participate in ongoing educational programs.
