Job Title: Social Worker

Department: Psychosocial Oncology

Job Summary: We are seeking a skilled Social Worker to join our team in providing assessment, service navigation, and case management services to cancer patients and their families. The successful candidate will work closely with our Program Manager and Clinical Lead to provide support and resources to those in need.

Key Responsibilities:

  • Provide assessment, service navigation, and case management services to cancer patients and their families
  • Develop collaborative relationships with patients, families, and internal/external departments for the delivery of support services
  • Liaise and collaborate with internal/external departments, social services, health organizations, and community resources to support clients/patients
  • Provide psychosocial counseling, crisis intervention, and referral and discharge planning as needed

Qualifications:

  • Post-secondary degree in Social Work from an accredited educational institution
  • 2 years of related social work experience in the last five (5) years

Skills and Abilities:

  • Expertise and experience with interprofessional collaboration, psychosocial assessment, consultation, and team building
  • Extensive knowledge of existing community resources and agencies providing relevant services to patient/clients/families
  • Demonstrated ability to promote participation, collaboration, and partnerships
  • Excellent assessment, interviewing, and counseling skills
  • Good organizational, time management, and prioritizing skills
  • Advanced communication skills, both written and oral

Physical Demands and Working Conditions:

  • May be required to hold a valid Manitoba driver's license and have access to a reliable vehicle
  • May encounter aggressive and/or agitated client/visitors/staff
  • May be occasionally required to work extended hours

Asset:

  • Experience working with children and families
  • Experience working with Child and Family Services

Other:

  • Must have satisfactory employment record
  • Must be legally able to work in Canada
  • Ability to speak French is considered an asset
