Job Summary: We are seeking a highly skilled and compassionate Registered Nurse to join our PRIME Program team. As a Registered Nurse, you will play a vital role in providing high-quality care to community-dwelling seniors, supporting them to continue living in their homes.

About the Role: The PRIME Program is a community-based health program dedicated to supporting seniors to live independently in their homes. As a Registered Nurse, you will work collaboratively with a multidisciplinary team to provide comprehensive care, including day centre services, primary health clinic, case management, and home care coordination. You will be responsible for assessing participants' needs, developing care plans, and providing education and support to participants and their families.

Key Responsibilities:

  • Provide high-quality, patient-centred care to community-dwelling seniors
  • Assess participants' needs and develop individualized care plans
  • Collaborate with the multidisciplinary team to provide comprehensive care
  • Provide education and support to participants and their families
  • Participate in quality improvement initiatives to enhance patient care

Requirements:

  • Graduate of an approved Registered Nursing educational program
  • Current registration with the College of Registered Nurses of Manitoba (CRNM)
  • Recent related experience with chronic illness and chronic care management, geriatrics, and complex continuing care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Ability to prioritize care for multiple participants

Preferred Qualifications:

  • Canadian Nurses Association (CNA) certified nurses
  • Advanced preparation in gerontology
  • Experience working with community-based health programs
