Key Responsibilities:
- Assess clients' needs and develop individualized care plans
- Coordinate care with healthcare professionals and community resources
- Provide education and support to clients and their families
- Collaborate with interdisciplinary teams to ensure comprehensive care
Requirements:
- Bachelor of Social Work (BSW) or related degree
- Valid Class 5 driver's license
- Experience working with mental health and addiction clients
- Knowledge of assessment, triage, treatment, and case management
- Knowledge of community resources and services in the clinical area
- Knowledge of crisis and suicide intervention and prevention programs
Preferred Qualifications:
- Registered/eligible for registration with the appropriate licensing body
- HSAS membership
