About the Role

We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at Fraser Health Authority. As a Patient Care Coordinator, you will play a critical role in providing exceptional care to patients with mental health and substance use issues.

Key Responsibilities
  • Assessment and Intervention: Conduct thorough assessments of patients' mental health and substance use needs, and develop individualized care plans to address their unique requirements.
  • Crisis Intervention: Provide crisis intervention and stabilization services to patients in emergency situations, ensuring their safety and well-being.
  • Collaboration and Communication: Work closely with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless care coordination and communication.
  • Documentation and Reporting: Accurately document patient assessments, care plans, and interventions, and provide regular updates to healthcare teams and families.
  • Education and Training: Participate in ongoing education and training to enhance your skills and knowledge in mental health and substance use care.
Requirements
  • Education and Experience: Graduation from an approved School of Nursing, plus current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year recent related experience in an acute or community Mental Health & Substance Use setting, or an equivalent combination of education, training, and experience.
  • Skills and Abilities: Ability to complete initial and ongoing mental health and substance use patient assessment and provide nursing care through appropriate/prescribed technical, therapeutic, safety type interventions including crisis intervention. Knowledge of mental illness and substance use treatment. Knowledge of psychopharmacology (indications and side effects). Counselling and group facilitation skills. Knowledge of legislation and government policies and procedures that relate to mental health and substance use practices. Ability to communicate effectively verbally and in writing with patients, families/care givers, the public, medical staff and members of the interprofessional care team.
