Mental Health Liaison Practitioner

The post holder will work as part of the MHLT to provide a psychiatric liaison service into Calderdale and Huddersfield Foundation Trust (CHFT) sites, covering both Calderdale Royal Hospital and Huddersfield Royal Infirmary. They will provide skilled assessment, support, advice and guidance for service users within CHFT who are presenting with a mental health concern. They will work shifts as part of the 24-hour staff rota to ensure the service is accessible at all times and that referrals are received and responded to within the commissioned response times. This job description outlines the main duties of the post.
The post holder will be required to undertake other duties as required with the grade as directed. The content of this post will be reviewed in consultation with the post holder when necessary and in line with service developments.
Key Responsibilities:
- Completing robust biopsychosocial mental health assessments with service users and formulating a safe and appropriate plan for discharge or further intervention and follow-up.
- Completing and documenting robust risk assessments in accordance with Trust/Organisational protocols.
- Ensuring that service users are actively involved in the planning of their care, including the involvement of families and carers.
- Working collaboratively with other professional groups and partnership organisations (Police, Citizens Advice, Benefits Agencies, Employment and Housing Associations, Education Authorities, etc.).
- Maintaining a continuing knowledge of current evidence-based practice.
- Maintaining relevant professional body registration and ensure adherence to statutory codes of practice.
- Contributing to ongoing service evaluation and development of the team.
- Recognising and respecting the cultural and spiritual needs of others.
- Engaging in clinical supervision and professional development review processes.
- Maintaining records in line with professional standards and Trust Protocols, ensuring confidentiality of information regarding service users and their families.
- Providing professional advice and guidance to colleagues within the acute trust around the management of individuals presenting with a mental health concern.
- Involvement in regular clinical team meetings and MDT care plan reviews to develop and implement individual care pathways.
- Involvement in the induction and orientation of students and new staff members.
- Adhering to all Trust Policies and Procedures, particularly the Trust's Code of Conduct for staff.
- Assisting the Team Manager in monitoring and evaluating service provision.
- Acting as a professional role model at all times and contributing to providing a positive learning environment for students.
- Being responsible for personal development agreed with the Line Manager.
- Regularly attending and contributing towards team meetings.
