Position Summary

The Practice Performance Manager (PPM) establishes trustworthy relationships with providers, and their clinical and support teams for the purpose of working directly with leadership and support staff of assigned provider practices in the Equality Health Network to deploy the Equality Health Care Model. The main role of the PPM is to enable and enhance practice performance from a reactive model of patient care to a proactive accountable care, analyzing population data to effectively measure, monitor, and manage care of patients and present results and outcomes to executive leadership.

Responsibilities

The Practice Performance Manager (PPM) is responsible for supporting assigned practice clinical teams in implementing the Equality Health value-based care program in the following key areas:

  1. Improve care of high risk, complex patients. Support practices in the effective use of CareEmpower Chart Prep Tool to manage high risk, complex patients and ensure care management with consistent visits to their PCP every quarter.
  2. Reduce avoidable admissions and readmissions. Support practices in the effective use of CareEmpower to track and manage care transitions of all inpatient discharges and ensure follow up with PCP and Specialist within 7 days. Measure and monitor success of outreach and develop strategies to simplify processes and ensure optimal care for patients.
  3. Improve preventive Wellness Visit rates. Collaborate with the practice quality and management teams to set weekly expectations on adult and child wellness visits. Identify and address barriers to bringing patients in and closing current HEDIS measures and care opportunities.
  4. Deploy Care Empower, Equality Health’s provider technology enablement and care gaps notification platform.
  5. Facilitate implementation of Equality Health’s digital data retrieval and integration (Datavant/Health Jump) with compatible practice EMRs.
  6. Work with assigned practices to drive change by assessing current workflows and assist practices in establishing improved care flows to ensure their high risk, complex patients are regularly seen and managed by their PCP; adults and children who are post-IP discharged are care transitioned back to their PCP within 7 days of discharge; and adults and children are proactively outreached and seen for their wellness visits and their gaps in care addressed and closed.
  7. Establish a regular meeting and operational touchpoint cadence with assigned practices to ensure progress on practice transformation and performance activities towards annual wellness, transitions of care, and high risk, complex patients care management goals.
  8. Leverage EQ Network Development Management, Market Medical Director, Clinical Quality, and Community Health Worker (CHW) Teams for our practices and the Equality Health network.
  9. Deliver monthly performance summary reports to the practice/providers and utilize data analysis techniques to convey opportunities for remediation and influence needed practice changes.
  10. Responsible for accurate documentation and maintenance of provider and practice demographics and attributes into the EQ client relationship management platform (Salesforce).
  11. Facilitate Cultural Care, HEDIS, STARS, correct coding, and RAF practice trainings as needed.

Required Knowledge, Education, & Experience

  1. Bachelor’s degree in business, healthcare administration, or a related field of study; or an equivalent combination of education and/or experience in healthcare delivery systems.
  2. A minimum of three (3) years of experience in the healthcare industry, preferably within provider practices.
  3. Excellent verbal, written and interpersonal communication skills.
  4. Highly collaborative team approach to work.
  5. Change management experience or exposure.
  6. Electronic Health Records experience or exposure.
  7. Proficiency with Microsoft Office applications and web-based technologies.
  8. Ability and willingness to travel to assigned practices and throughout the market service delivery area (SDA) as needed, upwards of 60% via personal automobile; occasional overnight stays may be required.

Highly Preferred Skills, Abilities, & Qualifications

  1. Familiarity with patient and practice risk adjustment mechanics, APR; HCC/RAF.
  2. Demonstrated ability to engage providers and support staff and drive performance to achieve a mutual patient population and care management goals.
  3. Ability to employ critical thinking to identify a problem, assess, then vision out the strategy and course of action to address then resolve it.
