The primary role of the Manager, Family Medicine Teaching Unit and Integrated Care Services is to lead and support clinical programs focused on activities in primary care, post-acute settings and the community.


This position will play a key role in the opening, growth and development of a primary care clinic to support the launch of an FMTU, an official teaching site of the University of Toronto's Department of Family and Community Medicine in collaboration with internal and external stakeholders,.

The manager will have collaborative relationships with physicians, the interprofessional team members, the clerical staff, residents and learners, community partners, as well as other members of HRH.

This role will provide operational leadership and direction in the delivery, availability, and continuous improvement of interdisciplinary clinical programs and services that provide patient-centered care.

The manager will lead quality improvement initiatives with the focus on integrated care and ED avoidance.

Responsibilities:

  • Monitor and evaluate daily operational clinical performance to optimize the quality of patient care delivery in alignment with organizational goals and priorities
  • Develop and monitor variances in unit operating budget in collaboration with the Director and Advance Business Intelligence(ABI)
  • Provide operational and clinical processes guidance to staff; manage direct and indirect reports in collaboration with interdisciplinary teams (e.g. Emergency Department, OHT and community partners, medical staff, regulated health professionals, etc.)
  • Promote and encourage a collaborative staff and patient environment for patientcentred care that adapts to the needs of diverse internal and external communities
  • Collaborate with inpatient and outpatient managers to identify process improvement strategies to improve transitions, reduce patient length of stay and prevent avoidable ED visits
  • Lead quality improvement as part of the portfolio, including implementation of new initiatives, pathways and clinics
  • Manage internal and external communications and coach unit staff in customer service methods; resolve patient and family concerns and labour related issues
  • Develop, plan, and implement innovative models and ideas for new areas of practice that support program strategies and enhance quality of patient care delivery, revenue generation, and cost effectiveness (e.g. approaches to reduce ED visits) in consultation with the Director, physicians, community health partners and stakeholders
  • Prepare project proposals, briefing notes, and business cases; conduct feasibility studies to determine project delivery methods and assign staff, equipment, and financial resources
  • Lead and participate in operational committees, both internal (e.g. Pharmacy Therapeutics, Safe Medication Practices) and external (professional clinical and operational practice networks including SCOPE Operation meetings)
  • Other duties as assigned

Requirements:

  • Graduate Degree in a Clinical Discipline, Health or Business Administration required
  • Regulated Health Care Professional in good standing with respective health professional regulatory body in Ontario
  • 5 years experience in a related area of clinical practice
  • 3 years experience in management/leadership
  • Membership with Nurses Association of Ontario (RNAO), preferred
  • Certified Health Executive (CHE), preferred
  • Basic Cardiac Life Support (BLCS) Certification, preferred
  • Canadian Nursing Association (CAN) certificate in area of clinical focus, preferred
  • Proficiency in MS Office (Outlook, Word, Excel, PowerPoint, etc.)
  • Working knowledge with Resource Matching & Referral (RM&R) system
  • Experience in using risk management strategies
  • Experience in project coordination and management, innovation, problem solving, leadership, relationship building, interpersonal, negotiation
