Are you guided by the principles of Housing First and passionate about supporting individuals in maintaining housing options based on their personal goals? The Intensive Case Management Team is part of the service delivery team of Homeward Trust Housing First in Edmonton that empowers individuals through a collaborative exploration of a person's strengths, barriers, interests, and goals.

You will build and maintain trusting partnerships with landlords, community service providers, and program participants in navigating affordable housing options and income funding.

Follow-up Support Workers create individual service plans, manage a caseload of 17-20 program participants, and provide ongoing support to participants through regular home visits.


  • What you bring to the team:
  • You bring 1+ years of related direct service work experience with vulnerable people.
  • You possess a diploma in Social Work, Human Services, or a related field.
  • You have a proven ability to practice relationshipbased approaches.
  • You are able to embed in your work the practices of harm reduction, personcentered, traumainformed, and strengthbased care.
  • You are able to remain confident and calm. This work may involve deescalating crisis situations with individuals under the influence of drugs or alcohol or experiencing a mental health crisis.
  • You have experience using evidencebased models of practice.
  • You understand and are committed to Housing principles.
  • You are able to use your personal vehicle to visit different locations, activities, or appointments throughout the Edmonton area.

Important work you will do at e4c:

  • You will work with the person served to build Individual Service Plans based on participants' identified strengths, barriers, interests, and goals.
  • You will use your knowledge of community and income resources to help the person served to maintain housing and make improvements to their overall wellbeing.
  • You will help individuals develop and connect with their natural support system. Providing mentorship and guidance as needed.
  • You are comfortable with difficult conversations and can use a trauma informed lens to help individuals understand the conditions of their lease agreements and maintain housing.
  • You will act as a mentor and advocate for the relationship between landlord & tenant and help to address concerns and mitigate the risk of eviction.
  • You are a natural promoter and know what it takes to build and maintain relationships with property managers, organizations, and landlords who are working with the ICMT program.
  • You will use Effort to Outcome software and regularly complete Service Prioritization Decision Assistance Tool (SPDAT) assessments and provincial assessments.
  • You know what it takes to build holistic relationships. You can connect to people from a variety of backgrounds.
