Support Worker

Reporting directly to the family the Inclusion Support Worker is responsible for providing personal support to our family member to enable them to be a valued, contributing member of the community.

Guided by the goals and objectives determined in collaboration and consultation with the family and individual, the Inclusion Support Worker will be responsible for supporting the development of skills and competencies that will assist our family member to pursue and fulfill their goals and aspirations.

Similarly, the Inclusion Support Worker will be responsible for liaising with a variety of community entities (employers, clubs, recreational groups) to support those entities in their efforts to engage our family member.


Key Responsibilities:

1. Connecting to Community

  • Assist the family and individual to identify places in the community where an individual can connect around a common area of interest i.e. recreational clubs, volunteer interests, associations
  • When necessary be the conduit between the community group and individual to facilitate the individual's meaningful participation
  • Provide support to the individual to enable them to attend, participate and be part of the community group(s)

2. Supporting Independent Living

  • Assist our family member to manage their finances, i.e. making spending decisions, performing banking functions, etc.
  • Support individual to collaboratively plan around household purchases
  • Collaborate with the individual around menu planning, shopping for groceries and cooking.
  • Encourage and influence a healthy lifestyle i.e. promote healthy eating, incorporating exercise into daily routines, ensuring any medications are used safely

Job Types:
Part-time, Casual

Part-time hours: 20-25 per week

Salary:
$18.00-$19.00 per hour

Schedule:

  • Evening shift
  • Monday to Friday
  • Weekend availability
