Reporting to the Program Manager, the Mental Health Clinician will be responsible for supporting clients in the community as a part of our Assertive Community Treatment Team.

The ACT Team is a community based program that is designed to stabilize individuals with severe and persistent mental illness who require ongoing, comprehensive, and individualized mental health services.


The Mental Health Clinician will provide standardized assessments, develop treatments plan, and participate in weekly meetings to provide assertive outreach for clients who have difficulty maintaining contact with mental health services.


Duties and Responsibilities:

  • Work in collaboration with the program psychiatrist to implement treatment plans
  • Monitor client's symptoms and initiate interventions to facilitate stabilization
  • Develop individual wellness plans to support complex physical and mental health needs
  • Establish and implement recovery and crisis plans
  • Participate and administer comprehensive psychiatric assessments or standardized assessment/screening tools where warranted (i.e., OCAN, GAINS, MSE, Wellness Plan)
  • Document client progress to maintain a record of activity
  • Participate in weekly staff meetings and treatment/service planning reviews
  • Provide on call support as necessary
  • Partner with clients and their families as members of the health care team
  • Support good health through the lens of the social determinants of health
  • Identify risk factors to decrease harm by establishing and implementing recovery and crisis plans
  • Liaise with community support and partners (i.e. mental health and addiction service providers, primary care, and family members)
  • Work independently and collaboratively within a multidisciplinary team to carry out mental health recovery and support
  • Understand and be able to integrate a clientcentered care and Housing First philosophy into daily responsibilities
  • Demonstrate a strong commitment to quality care and service delivery based on SHIP's Mission and Values
  • Ensure appropriate records are kept and make sure that statistical information is maintained as per agency standards
  • Adherence to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
  • Perform additional duties as required/delegated by Management

Qualifications:

  • Postsecondary education in Social Work, Social Services, Health Services, or equivalent
- +2 years experience supporting individuals with mental health challenges in the community

  • Knowledge of psychiatric symptomology, and suicide assessment/intervention
  • Demonstrated knowledge in psychosocial assessments, mental illness, counselling, and crisis management
  • Knowledge of the Mental Health Act, housing, social, financial, vocational, and other community resources
  • Demonstrated cultural competence and the ability to support a diverse client population
  • Knowledge and commitment to the values of psychosocial rehabilitation and recovery oriented practices
  • Expertise in collaboration with the community in a multidisciplinary team while supporting the mental health population
  • Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
  • Excellent oral and written communication skills
  • Strong interpersonal, problem solving and organizational skills
  • A valid Ontario Class "G" Driver's license and a vehicle with adequate insurance coverage is required
  • Valid certification in CPI, CPR, First Aid, and ASIST

Working Conditions and Physical Requirements:

  • Hours typically rotate Monday
  • Friday, from 8:30am 4:30pm and 11:00am7:00pm each week but may vary depending on client needs and includes oncall responsibilities on a bimonthly rotation
  • Average amount of physical effort is required.
  • Able to sit/stand for prolonged periods of time
  • Able to answer and communicate telephone calls and general computer use
  • Able to lift up to 20lbs (product, documents, etc.)
  • Vehicle travel to sites for visits/walkthroughs and open doors as required
