We are excited to announce that we are seeking a HCA OR LPN to join our team as the Care Coach/Supervisor to support the Health Care Aides and clinical team.

Hamilton Village is centrally located in East Richmond and only a short 5 minute drive from the Alex Fraser Bridge when commuting from Delta or Surrey. We are excited to announce that we are seeking a Care Coach/Supervisor to support the Health Care Aides and clinical team.


Reporting to the Director of Care (DOC), the Care Coach is an integral part of the care team and is responsible for monitoring and supporting the overall performance of the Health Care Aides, while delivering exemplary care in alignment with our five cultural pillars: Person and Family-Centred Care, Safety as a Priority, Quality and Innovation, A Great Place to Work, and Sustainability.


Responsibilities and Duties - Provide leadership and direction to the HCAs. - Ensures HCAs are in strict compliance with the HCA Job Routines. - Provides input into resident care quality improvement goals in collaboration with the ADOC, DOC and the Interdisciplinary Team. - Must be willing to work on evenings and nights as needed to support HCAs. - Measure and analyze service level performance results and make proactive adjustments based on results. - Provide recommendations and input to operating policies and procedures based on industry best practices and current practice guidelines to achieve greater efficiency and exceptional resident care. - Engage with residents and families to contribute towards care plan development with the Interdisciplinary Team. - Communicate weekly and monthly resident care goals in such a way that it inspires and motivates the team to achieve greater quality and efficiency. - Provide demonstrated coaching and training to team members. - Refine performance expectations for all employees to reflect improvements that have been implemented. - Through hands on assistance and support, monitor staff's clinical skill levels, develop an improvement plan and implement corrective measures to ensure compliance with established care standards. - Coordinates and leads orientation for new HCA hires. - Ensures ADLs are reviewed monthly and reflects current residents' care needs as indicated in the nursing care plans (i.e., bathing schedules, bathing preferences, etc). - Make appropriate adjustments to HCA schedules/assignments in response to the residents' changing care needs. - Complete performance evaluations on all HCAs prior to the end of their probationary period, and annually thereafter. - Ensure that staff review the designated policies as part of each performance review and verify the review in writing. - Manage the day-to-day operations of residential care within the scope of the contract. - Ensure compliance with all government, employment and industry laws, regulations and requirements by staying informed of changes and their impact on the organization and its goals. - Proactively meet with residents and their families to discuss, provide support and guidance regarding the resident's plan of care. - Ensure that established standards of care are communicated to staff and consistently met. - Work with the staff team and leadership to resolve conflicts should concerns arise. - Attend facility committees as required. - Ensure regular monthly HCAs meetings are held as required and minutes are taken. Minutes must be distributed to staff. - Ensure all audits (i.e., monthly Compliance Audits, Care Audits, & Hand Hygiene Audits) and Human Resource audits are completed as required and posted on SharePoint. Develop and post an action plan with target dates to address areas of non-compliance following discussion with the Director of Care and General Manager. - Ensures that HCAs complete resident care and tasks on the unit such as daily care, supervision of meal services (including in room), re-stocking supplies, reprocessing, and maintenance of equipment. - Corrects unsafe work conditions immediately and reports to Director of Care for further corrective actions. - Participates in on-going formal/informal education. - Familiar with policies and procedures of the facility and ensures 100% compliance and provides input with development. - Participates with licensing inspection of the facility, a review of the operations, enforcement of policies and procedures in all matters of care. - Participates with infection prevention and control measures (i.e., reprocessing procedures). - Participates in the OH & S committee and
