Program Overview - First Step to Home, Community Care_**- First Step to Home (FSTH) is a transitional support program with housing for men 55 years and up who have been housing insecure or street involved, experiencing mental health and/or addictions issues, and geriatric challenges.

First Step to Home provides transitional housing in furnished bachelor units as part of program participation.

The First Step to Home team is comprised of health care professionals who work collaboratively, within a harm reduction and recovery-based framework to support participants to gain skills needed to make a successful transition to a more stable, healthier and independent lifestyle.

What You Will Do_**- _

Interact with individuals in the program in person, providing emotional support and feedback and encouraging and facilitating their participation in the program.

_
- _ Provide one to one support to individuals as outlined in their Coordinated Care plans, supporting the social worker with implementation of transactional case management activities including form filling and navigating formal systems._
- _ Support intake process through applicant selection process, interview participation, intake management and follow up with new participants._
- _ Monitor participants' progress and well-being and assists the team in evaluating the effectiveness of Coordinated Care Plans, providing suggestions for modification as required._
- _ Inform goal setting and Coordinated Care planning process._
- _ Plan, create, and implement relevant individual and group programming with participants. _
- _Assist participants to function more independently while in the program and the community. _
- _Use the ADL/IADL tool on a regular basis. _
- _ Engage in skill development to assist individuals to carry out activities of daily living such as grooming, hygiene, shopping, banking, and informal safety training and/or behavior management as outlined in Coordinated Care plans._
- _ Support individuals to carry out activities that enhance their independence and participation in the community, identifying services and activities that will meet the participants' needs and provide support, guidance, and instruction to facilitate successful participation._
- _ Liaise with other service providers and the broader community to build and establish networks._
- _ Recognize and assess potential aggressive behaviour or crisis situations and develop strategies to deal with such situations and inform team and Management when such incidents arise. _
- _Perform delegated tasks as directed by health care professionals, such as managing medication for individuals in accordance with primary care instruction. _
- _Arrange transportation for participants and may accompany individuals to appointments and community activities when necessary._
- _ Complete and maintain documentation and case note entries efficiently._
- _ Maintain up-to-date knowledge of internal fire regulations and procedures, and other emergency procedures and ensure health and safety standards are maintained. _
- _Provide input to the development of the program and participates in program evaluations._

What You Bring to the Team_**- _
Social Service Worker diploma or other related education._
- _ 2+ years of experience working with older adults and/or seniors with mental health, addictions issues, and street involvement._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._
