The Cedarvale Supportive Housing Program offers housing for seniors with a history of street involvement, and potential mental health and/or substance abuse challenges. The program delivers psychosocial and primary care services on-site, including intensive case management, addiction support, counseling, nursing care, life skills development, and social activities.

What You Will Do
  • Interact directly with program participants, offering emotional support and encouragement to engage in program activities.
  • Provide personalized support according to Coordinated Care plans, assist with case management tasks, and aid in the intake process for new participants.
  • Monitor progress, contribute to care plan effectiveness evaluation, and suggest modifications when necessary.
  • Participate in goal setting and planning processes, devising and executing individual and group activities.
  • Help individuals enhance independence in everyday tasks and within the community.
  • Collaborate with service providers and the community to build networks and support participants.
  • Recognize and address potential crisis situations, apply strategies for resolution, and maintain documentation efficiently.
  • Contribute to program development and evaluations.

This role may involve evening and weekend shifts, exposure to various elements, and potential challenging behaviors.

What You Bring to the Team
  • Completed Social Service Worker diploma or related education.
  • Minimum 2 years of experience with older adults facing mental health, addiction issues, and street involvement.

  • Ability to engage effectively with high-risk and vulnerable individuals.
  • Experience in managing aggressive behaviors and strong communication skills.
  • Capability in conflict resolution, crisis intervention, and decision-making.
  • Proficiency in computer use, communication, and understanding of relevant topics.
