Nurse Clinician Case Manager, Complex Developmental Behavioral Conditions (CDBC)

As a member of the interdisciplinary team, the Nurse Clinician Case Manager Complex Developmental Behavioural Conditions (CDBC) will coordinate the development, implementation and evaluation of service delivery plans for patients and their families. This includes activities such as patient intake and referral triaging, liaising with Sunny Hill and BCCH staff and community-based health care professionals/agencies, coordinating and facilitating teaching for patients/family and staff in-servicing, in a manner that is consistent with established policies, standards and philosophies of Sunny Hill Health Centre for Children and British Columbia's Children's Hospital.

What you'll do

Initiate and coordinate assessments, respond to patient, family and health care member's enquiries, providing anticipatory guidance and case management, oversee clinic bookings and liaise with other support diagnostic/health services in coordinating patient referrals. Direct the intake/access process for a team of the CDBC Program by performing duties such as receiving, collecting and documenting child specific information related to developmental and behavioural concerns, conducting interviews and problem-solving with families and other team members, assessing the urgency and appropriateness of the referral, triaging as necessary as well as managing the waitlist. Facilitate the re-direction of referrals to other community resources, and attend intake/access meetings as required. Collaborate with other members of the health care team to evaluate family-centered patient management in the particular setting by performing activities such as reviewing patient/family history, developing individualized service plans for specific patients/families, and meeting with other members of the health care team to discuss and review the recommendations. Coordinate teaching of patients/families with a new developmental and/or behavioural diagnosis including the development of teaching resources, referring to and liaising with other health care professionals, acting as a resource for other health care professionals, identifying learning needs and conducting teaching sessions. Provide supportive case management to families by ensuring they receive assistance with patient advocacy, information regarding program, team, or community health and educational resources, and referrals to appropriate community services and/or agencies. Act as a liaison, as appropriate, between the program, the interdisciplinary team, and community health and educational services and agencies to clarify information, responsibilities and recommendations to ensure the needs of the patient/family are addressed. Establish a collaborative working relationship with Sunny Hill and BCCH staff and community-based health care and service providers. Liaise with appropriate community and professional groups, nursing staff and agencies, regarding Complex Developmental Behavioural Conditions through consultation and teaching. In collaboration with the CDBC Program Manager, develops policies, procedures and operational requirements related to the function of the CDBC teams by observing service needs and providing plans that address patient and family needs, clinic operations and promotes effective use of resources.

What you bring

Qualifications

Graduation from an approved School of Nursing with current practicing registration as an RN with the British Columbia College of Nurses and Midwives (BCCNM). Three years recent related and progressively responsible experience including previous experience working with the population and/or in a child development and rehabilitation setting or an equivalent combination of education, training and experience. CPR Training – Basic Cardiac Life Support (BCLS) – Level I. Valid BC Driver's Licence and access to vehicle and ability to travel.

You will also have:

Knowledge of community based services for children with developmental behavioural conditions. Excellent communication skills, both oral and written. Strong leadership and organizational skills. Ability to relate effectively to children and their families, dealing effectively with those experiencing stress and /or in crisis. Ability to collaborate and work effectively as a member of an interdisciplinary care team and function as a nursing resource. Ability to plan, deliver and evaluate educational sessions based upon principles of adult education. Ability to identify and resolve problems using therapeutic communication strategies. Ability to work independently to organise and prioritise multiple tasks. Physical ability to perform the duties of the job. Strong computer skills, including Excel, Word and PowerPoint and Internet Browsers.
