Guided by St.

Joseph's strategic plan, the Coordinator of Medicine Services will support building regional coalitions that will reach out to the people who need us most and close gaps in care.

The incumbent will work to ensure that excellent care is accessible to everyone and will welcome every patient and family caregiver as a person with a unique set of strengths, support networks, spirituality, physical and psychological conditions, and personal goals.


The Coordinator of Medicine Services is responsible for the day to day operation and the coordination of chronic disease management services with the clinical teams in collaboration with Medical Leadership.

These clinical services and programs include General Internal Medicine, General Cardiology, Respirology, Asthma, Allergy/Immunology, Infectious Diseases Care Program and the Pulmonary Function Lab.

Reporting to the Director, the Coordinator utilizes clinical and leadership skills to ensure quality patient care through the planning, operational management, and coordination of human, financial and physical resources.

In addition, the incumbent will play a leadership role in developing St.

Joseph's priority of being a leader in the provision of Chronic Disease Management by creating linkages across programs and partner across the region to connect care and services to meet the needs of the whole person.

The incumbent will work collaboratively with regional health providers, hospitals, non
- governmental organizations, Home and Community Care and South West LHIN leaders in building a more effective care system. Program development and evaluation are integral to this role.

Essential Qualifications

  • Successful completion of a Bachelor of Science in Nursing or Allied Health Profession
  • Valid registration as a regulated health professional with the associated professional College
  • Minimum of 5 years experience in a health care leadership role
  • Previous leadership experience in a unionized environment
  • Demonstrated experience in program development, research and evaluation, project management and LEAN training
  • Demonstrated knowledge of ambulatory care and chronic disease management models
  • Excellent decisionmaking skills with the ability to form/defend independent judgments and to use good judgment in assessing difficult situations.
  • Ability to work effectively and demonstrates composure under pressure/stressful conditions and to meet multiple and competing deadlines
  • Demonstrates strong leadership and motivational skills
  • Exercises initiative and good judgment with ability to multitask
  • Adapts to and implements change and facilitates its acceptance by others
  • Has knowledge of relevant legislation related to management of people and fiscal resources
  • Demonstrated shared leadership, team development and collaboration, and facilitation skills coupled with superior interpersonal, relationship and communication skills
  • Demonstrate and practice the values of St. Joseph's in daily interactions with staff, patients and families
  • Demonstrated critical thinking, problem recognition, analyses and resolution skills
  • Experience working with outside health agencies, government and nongovernmental agencies
  • Demonstrated abilities in fiscal management and operational decisionmaking
  • Ability to work independently as well as collaboratively with a wide range of professionals
  • Demonstrated interest and experience in research and program evaluation
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA

Preferred Qualifications

  • Proficiency in French would be an asset
  • Masters degree in Business Administration or Health Administration

Immunization Requirements

  • Provide documentation you have received two doses of the Covid19 vaccine (primary series, boosters and/or XBB) OR one dose of XBB vaccine at least 14 days prior to the start date.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
