QUALIFICATIONS:

Bachelor's degree in Social Work, Child & Youth Development or a related discipline. Further education/certification in Cultural Studies, Gender & Women, African, Indigenous Studies or a related discipline would be an asset. Training and/or certification in Conflict Management and Resolution, Leadership would be an asset. Minimum - 5 years of experience working in a trauma-informed practice. Experience working with individuals with developmental disabilities. Experience working in areas of FASD, Mental Health, Dual Diagnosis, Substance abuse, addictions and the Justice System is preferred. In-depth understanding of the principles of Harm Reduction and Trauma based approaches. Prior experience in staff/parent/student training. Strong knowledge in behavioural/developmental services.

Proficiency with technology including, but not limited to computers including MS Office (Excel, Word, Outlook), and other software programs needed to produce documents, reports, databases, presentations and communication vehicles for staff learning and development.


JOB SUMMARY:


The Supported Living (SL) Facilitator is accountable for establishing a working relationship with adults 18 years of age or older who are identified as a Youth entering Supported Living through the Child Welfare and Shelter system or an individual who meets the criteria outlined.

Maintaining a caseload of individuals including new and potential referrals. The SL Facilitator is responsible for developing and supervising the use and implementation of programs and assessments for individuals. Works collaboratively with staff, families, clinical teams, professional supports, and community agencies, providing leadership in coordinating ongoing support. Responsible for preparing ongoing documentation as required by relevant stakeholders.

the SL Facilitator identifies and facilitates access to generic community supports as well as government-funded services and supports (e.g., Passport Program, Legal Aid, etc.).

The primary focus is delivering support, timely and responsive case management, and advocacy services through regular face-to-face meetings and collaboration, support and guidance to the internal Support Team working, with a goal of improving an individual's overall quality of life including independence, stability, and social inclusion.


RESPONSIBILITIES:

Case Management

  • Works with individuals to develop a trusting, respectful relationship in order to understand their strengths, areas for development, and goals.
  • Encourages active participation and selfdetermination by the individual as it relates to setting and working towards their goals.
  • Assists in clientcentred planning and advocacy, assessing individuals' strengths and needs, determining natural existing supports as well as availability and appropriateness of community and professional supports.
  • Supports the assigned support team (Community Support Workers & respective Support Workers) in their provision of ongoing comprehensive case management to individuals, assessing, planning, implementing, coordinating, monitoring and evaluating the services and supports required to meet the individual's service needs.
  • Coordinates access to community resources, acting as liaison with other service providers to determine and address service issues and gaps.
  • Provides links to services including but not limited to: Legal services, Medical/dental care, Parenting support, Community partners, Social/recreational, Rehabilitation services.

Outreach

  • Works closely with community agencies, local mental health professionals, therapists, and medical and legal practitioners to provide individuals served with the best possible options.
  • Speaks on behalf of individual adults with developmental disabilities in their dealings with community agencies and services, institutions and associations, and government bodies as well as members of the public to ensure that their personal and human rights are not compromised. Initiates and advocates for any possible accommodations with external services that may enhance chances of success and/or participation directly related to the specific needs for that individual.

Advocacy

  • Assists individuals and strengthens their capacity to acquire the skills necessary to manage daily living.
  • Responds to, investigates, assesses, and makes recommendations in cases where individuals being served are in crisis and engages professional assistance where required.
