The Client Care Coordinator will work with clients with complex needs by engaging in psycho social assessment, trauma informed counselling and brief case management over 6-10 sessions.

This will include navigating the services available both within and outside SRCHC.

The Client Care Coordinator will work across three organizational teams (Integrated Primary Care, Substance Use & Mental Health, and Chronic Disease) as a member of a multidisciplinary team.

This position is expected to work onsite.

Responsibilities:

  • Engage in psycho social assessment, trauma informed counselling and brief case management with a range of clients
  • Act as a frontline community contact person and assist clients with complex needs in navigating the services available within SRCHC and the external health and social services and settlement sectors
  • Dependent on own team, responsible to provide outreach and support to newcomer communities, support to CATCH project, outreach clinic and women's dropin programs
  • Provide centralized intake support to new SRCHC clients with complex needs
  • Provide case management supports, crisis supports and systems coordination, navigation and community bridging to improve access to health, social, legal and other services for individuals and their families
  • Provide cross organizational oncall crisis response as required
  • Assist in locating required community resources by liaising with community groups and other health/social service agencies to provide referral information for clients, and to make referrals to other community health centre professionals to ensure a care continuum of health and social services
  • Ensure all available and required resources are in place and are accessed appropriately
  • Provide relevant shortterm counselling services as appropriate
  • Facilitate cultural interpretation for clients' appointments
  • Facilitate transports and/or accompany clients to medical appointments, court, community programs and other appointments
  • Advocate for clients on individual and systemic issues where appropriate
  • Maintain uptodate client files as specified by the recording procedure
  • Participate as a member in all team functions e.g. program planning, team meetings, chart reviews, case conferences, etc.
  • Collaborates within own team and other teams across the organization to identify and provide health education to target groups
  • All other duties as required

Qualifications:

  • Masters degree in Social Work or Bachelors degree in Social Work, coupled with relevant experience
  • Certification or eligibility for membership in Ontario College of Social Workers or Social Service Workers

  • Thorough knowledge of and proficiency in psycho social assessment, trauma informed counseling and case management, harm reduction techniques
  • Solid client, and systems advocacy skills and experience
- knowledge of community programs, health, social, legal and other service systems in Toronto as well as an awareness of and sensitivity to the health issues of a diverse community

  • A strong desire and experience working with undocumented individuals, with people who are homeless, living with mental illness or substance use and/or with complex health needs
  • Ability to deal effectively with people experiencing crises, mental health challenges and/or diverse cultural interpretations of health
  • Strong oral and written communication skills
  • Excellent diplomatic and interpersonal skills
  • Strong commitment to health promotion and community development
  • Proven skills in reframing potential conflict situations
  • Nonjudgemental / nonconfrontational attitude
  • The ability to use creative strategies that address systemic barriers, increase access to community resources and improve the quality of life of clients
