Job Description Summary

The Can-SOLVE CKD Network is a pan-Canadian network promoting patient-oriented research in chronic kidney disease. The network consists of a large and multi-disciplinary team across the country with a core operations team that supports day-to-day activities.

The activities and accomplishments of this network will benefit Canadians by promoting a cultural shift from care which is disease-centred and provider-focused to care which is patient-centred, informed by evidence, and coordinated across the health care system.

The Kidney Check project, one of Can-SOLVE CKD s 18 research projects, is part of a national program which will be run in several provinces. In BC, the project will be conducted in partnership the First Nations Health Authority. It is a province wide project aimed at provided kidney health screening, triage and treatment referral in selected First Nations communities.

The goal of this project is to establish a viable long-term kidney health wellness and disease screening and treatment program to reduce incidence of kidney health failure in First Nations communities. An understanding of and demonstrated respect of First Nations cultural principles and protocols, and being able to apply this understanding in work situations is an important component of this position.

Kidney Check Project Nurse will work in collaboration with the Kidney Check Project Coordinator, to coordinate and implement the project in BC. He/she will perform nursing duties, work collaboratively with local community health teams and provide training and mentorship to these teams to develop on-site capacity to conduct the program.

The position is a one-year contract role, approximately 28 hours per week, with the possibility of renewal and extension subject to funding and performance. Travel to BC communities will be required, potentially other study or network related travel as well.

Organizational Status

This position must work independently and within standard and acceptable boundaries for ethical and competent nursing practice. The position will be accountable to the Executive Director, Can-SOLVE CKD network.

Work Performed

  • Assists in the planning and development of community-based screening programs, ensuring the Can-SOLVE CKD / BC Renal screening protocols and referral pathways are implemented.
  • Perform primary screening and risk prediction (direct, point of care, instant feedback) for kidney health wellness and disease on First Nations individuals, using culturally safe engagement practices.
  • Communicate pertinent information such as lab results and the meaning of lab results to individuals, using culturally safe engagement practices.
  • Works with community-based teams to ensure there is a process for referrals to a higher level of care. Based on the incorporation of information and communication technologies such as advanced point of care testing equipment.
  • Implement a sustainable holistic care and referral process for people at higher risk of kidney failure immediately to kidney health clinics within the health authority region Kidney Programs (HARPS), Kidney Agency
  • Co-manage those at lower risk with local health care teams including community health staff.
  • Facilitate relationships with family physicians, specialists, and Community Health Program staff to ensure continuity of follow-up care for clients.
  • Engage in capacity building and/or knowledge transfer to local health care teams.
  • Mentor nurses working in the community to educate and coach clients on kidney health and wellness.
  • Mentor nurses working within the community to follow the recommended screening process.
  • Provide training on use of point of care testing and evaluation of screening results based established protocols to community staff (CHNs, LPNs, and other identified staff).
  • Mentor nurses working in the community to clean and check point of care equipment for purposes of quality assurance.
  • Evaluate the effectiveness of screening and referral services to ensure the project is meeting clinical targets (e.g., screening time, treatment time, duration between interventions),
  • Identifies problems along screening, treatment and follow-up care pathways.
  • Participate in the resolution of any problems identified along screening and referral pathways.
  • Ensure the security, confidentiality, accuracy and integrity of individual client records
  • Adheres to the restrictions deemed appropriate on the use and storage of data
  • Performs other related duties as assigned.

Preferred Qualifications

Research Nurse: Registered Nurse with BCCNP. Clinical Associate: Undergraduate degree in Nursing & Registered Nurse with BCCNP. Nurse Practitioner Clinical Associate: Postgraduate degree in Nursing & Registered Nurse with BCCNP. Nursing Licensure in British Columbia. Clinical Associates require Master's Degree in Nursing. Current practicing registration with the College of Registered Nurses of British Columbia Nursing (CRNBC). Current certification in basic CPR mandatory.Research Nurse: Three years of nursing and administrative experience. Clinical Associate: At least two years of experience in a clinical position. Nurse Practitioner Clinical Associate: Two years of experience in primary care NP practice or the equivalent combination of education and experience. Research Nurse: Three years of nursing and administrative experience. Clinical Associate: At least two years of experience in a clinical position. Nurse Practitioner Clinical Associate: Two years of experience in primary care practice or the equivalent combination of education and experience. Broad knowledge of nursing theory and practice within a client/community centered model of care. CNA certification in Nephrology {CNeph(C)} is an asset. Experience working with First Nations in a cultural safe manner. Knowledge of First Nations health issues and the healthcare system in British Columbia. San yas Indigenous Cultural Safety Course an asset (or willing to train). OCAP certification is an asset (or willing to train). Demonstrated ability to effectively deal with others in a consultative/collaborative and culturally safe manner.Demonstrated ability to organize and prioritize work and to work independently.
