Reporting to the Director, Rehabilitative & Transitional Services, this position brings strong leadership and management skills to teams working to build and sustain excellence at Trillium Health Partners.
Leadership Responsibilities
- Oversees daily operations of discharge planning to ensure quality of work
- Provides leadership to the discharge planning team in setting priorities for practice and quality improvement
- Works directly with Director to ensure concurrent reviews are timely and discharge plan development is in place
- Fosters collaborative communication with internal interdisciplinary teams and community partners regarding seamless progress towards discharge
- Ensures that all Discharge Planning policies are being followed
- Works collaboratively in partnership with program managers to ensure that program service needs are met and that resource allocation is appropriate
- Works with patients and families as partners in care; collaborates to continuously improve the patient experience
- Responds quickly and effectively to quality and risk issues and leads teams in improvement initiatives
- Reviews Program performance regularly, uses data to drive decisionmaking and improvement strategies, and leads teams to drive best practices to improve patient outcomes
- Collaborates with team members to create an effective employee engagement strategy
- Coaches and mentors health care professionals to navigate successfully through change
- Manages all aspects of employee relations including: staff development and retention; attendance and performance management
- Collaborates with Human Resources partners to develop and implement a responsive work force plan
- Supports strategic research, innovation, and education mandate at the corporate and program level
Qualifications & Skills:
- Master's Degree in a health related field
- Minimum three years relevant leadership experience in a health care setting
- Strong business management skills (strategic planning, budgeting, resource allocation, and analytics)
- Knowledge of relevant legislation, practice standards and best practice guidelines
- Excellent written and verbal communication and presentation skills
- Develop flexible, responsive system to meet changing demands
- Strong skills in conflict management; critical thinking, problemsolving, and decisionmaking
- Ability to function effectively and lead during periods of rapid change and transition
- Ability to remain calm under pressure; role models composure
Position Location and Travel:
- Travel between all sites of Trillium Health Partners will be required.
