POSITION SUMMARY:

We are seeking an experienced Project Manager to join the Model of Care (MOC) Initiative. Reporting to the Clinical Project Director, MOC, the successful candidate will be responsible for supporting the planning, implementation and scaling of the MOC project. They will work alongside key partners to supporting all aspects of the initiative, including the development of new tools and resources, preparation for and documentation of key work streams. This project manager will leverage project management skills and tools, data, evidence-based practice, change management strategies, and improvement science to ensure successful achievement of project targets and timelines. This work will be piloted in the Inpatient Medicine Program and scaled across the organization's inpatient platform. As part of this role, the successful candidate will facilitate the integration of MOC with Foundations of Clinical Excellence, maximize opportunities for collaboration, reducing duplication, and minimizing project risks and issues.

This role will also collaborate with team members and leaders in Foundations of Clinical Excellence, Quality, Risk, Practice, CSI, People Health, Finance, Labour Relations, Patient Care Services, Business Intelligence, Operational Performance, Master Plan, IT, and the Institute of Better Health as well as external partners where appropriate. This work will be pivotal in creating the conditions, processes and tools that enable our people to achieve consistent clinical excellence, wellness, and joy across the clinical platform.

We are seeking an Interim Project Manager to support the MOC project over 24 months

ACCOUNTABILITIES:

Oversee all elements of model of care workplan development and implementation including communication plan, milestone and status tracking, data collection and reporting etc. Function as a lead for Foundations of Clinical Excellence and Senior Leadership Committee reporting and support material development on the project workplan/status Monitor and report on progress, problems, and solutions of the project to all leaders and teams, escalating issues as appropriate Support project work streams and committee meetings, including process re-design, while simultaneously providing project coaching, motivation and guidance throughout the duration of the project Create and implement change management plans that maximize employee engagement, ensuring smooth adoption of changes impacting employees and the organization Engage and communicate with leaders and teams on an ongoing basis and ensure changing resource and project requirements are met and understood Participate in the creation and delivery of innovative presentations and proposals to all levels of THP.

FORMAL, MINIMUM EDUCATION REQUIRED:

Undergraduate degree in business, life sciences, health informatics or related discipline required Master's degree in public administration, business administration, or healthcare administration or equivalent experience preferred A certified professional in healthcare quality and/or improvement, or PMP designation, or Six Sigma Black Belt, or equivalent is an asset Change Management certification an asset (. PROSCI).

PREVIOUS RELATED OR SPECIAL EXPERIENCE NECESSARY :

Minimum 2+ years of experience in project management or leading complex change initiatives in healthcare environments such as hospitals, government or private sector Demonstrated success in supporting complex cultural change in healthcare environment preferred Excellent organizational, time management and project management skills Experience using data to influence action and results Experience with EPIC is an asset Superior analytical, critical thinking, and problem solving Demonstrated effectiveness at leading and working with large/complex teams Experience organizing and facilitating meetings and engagement activities with multiple partners Effective interpersonal communication skills and ability to successfully build relationships Demonstrated ability to multitask and prioritize tasks effectively Knowledge of systems integration Relevant experience in building and maintaining partnerships Excellent analytical skills to critically evaluate information gathered from multiple sources Superior writing skills to communicate information effectively to various audiences, with the proven ability to explain and communicate complex topics Demonstrated ability to anticipate needs of senior leaders and clinicians Demonstrated effectiveness at leading and working with teams Demonstrated ability to develop effective and comprehensive reports and presentations for various audiences, including executives, administrators, and clinicians Ability to perform duties under deadline pressure in a fast-paced environment Strong working knowledge of computer applications, especially the Microsoft Office suite Satisfactory performance and attendance required Ability to maintain confidentiality in sensitive situations required
