  • This employee will work in cooperation and alongside front desk staff, YWCA Hamilton staff, including assigned Program Supervisor, full time, parttime and relief team membersThey will perform all duties and responsibilities in a productive, professional and cooperative manner which will contribute to the overall goals of the Supported Apartment Program, the DS Department and in keeping with general policies and guidelines of YWCA Hamilton.
  • Ensure the physical and emotional wellbeing of individuals we support is maintained;
  • Follow and adhere to QAM compliance and Legislation 299/10 standards;
  • This employee will provide delivery of direct support services to individuals we support within their Supported Apartment Program;
  • Support services in all areas of daily living within our service plan domains including but not limited to: medication administration, chore lists/housekeeping, laundry, nightly safety inspections, and advocacy;
  • Must follow BSPs and PRN protocols. Provide supervision and ensure safety of residents during their shift. Support services offered will maintain or increase individual's independent living skills and personal growth;
  • In cooperation with program supervisor will perform all duties and responsibilities in a productive, professional and cooperative manner which will contribute to the overall goals of the program and DS Department and in keeping with general policies and guidelines of YWCA Hamilton;
  • Ensure the physical and emotional wellbeing of individuals we support is maintained;
  • Assist residents with ADL's, medication administration, assistance with hygiene routines, housekeeping, laundry, community safety skills, reducing risks to harm;
  • Must attend and participate in regular team meetings in order to communicate and share information including achievements, progress, concerns and generate solutions;
  • Attend ongoing professional development opportunities offered to program staff;
  • Program and agency policies are adhered to by staff and reviewed regularly;
  • All necessary administrative/documentation tasks are performed: ISP & Reviews, ITPs, BSPs, PRN Protocols adhered too, behaviour data collection, teaching & wellness plans, daily communication logs, checklists, chore lists, inspections, incident reports, fire drills or other reports/tasks as assigned;
  • Ensure that housekeeping/cleanliness/maintenance of program areas or residents' accommodations standards are met;
  • Establish positive professional relationships with staff, individuals, families, and other community service agency providers;
  • Provide accurate and timely reports as required;
  • Ensure YWCA Hamilton workplace health and safety policies and procedures are followed at all times and act to reduce workplace hazards;
  • Report all workplace hazards and follow YWCA Hamilton health and safety policies.

How to Apply

  • Minimum Education: Completion of College Diploma in DSW, SSW or DSW Apprenticeship;
  • 23 years' full time case management work experience in a 24hr residential setting;
  • Previous work experience with individuals with Mental Health Diagnosis and Challenging Behaviours;
