The Manager for Transition Services supports patient flow and provides leadership to the transition services team. Description:

The Manager of Transitions Services (TS) reports directly to the Program Manager for Site Operations. This position is responsible and accountable for overseeing the day-to-day operations of the Transition Services team and ensuring the effective and efficient management of human, material, and financial resources. The Manager of TS is accountable for optimizing patient flow from our acute care site to the most appropriate community options. Through the identification of barriers to discharge the Manager of TS works in collaboration with unit leadership, interdisciplinary team members, Zone and Community partners to resolve issues and address undesirable trends negatively impacting patient length of stay. Additionally, the Manager of TS collaborates with intersectoral resources to support and help plan complex discharges. Responsibilities include but are not limited to: Establishing and maintaining operational efficiency by identifying budget requirements for labor, supply, and equipment resources.Managing human resources including but not limited to: recruitment (hire, transfer, promotions etc.), disciplinary, grievances, coaching, mentoring, rotations, and performance management.Active involvement in program development by identifying service delivery improvements and acting as a change agent and resource for implementation of organizational changes.Pursuing ongoing personal and professional development.

Required Qualifications:

Baccalaureate Degree in Nursing – BN, BScN or equivalent.Active, or eligible for, registration with the College of Registered Nurses of Alberta (CRNA).

Additional Required Qualifications:

Knowledge and experience in adult acute care nursing, community nursing, and complex discharge planning.One year of community care coordination experience.Experience working as a Transition Coordinator and/or a strong understanding of the Transition Coordinator role, scope and responsibilities.Demonstrated understanding of appropriate placement of patients and the supports required for successful patient placement.Experience analyzing data, identifying trends, and making and implementing recommendations which contribute to decreasing length of stay or readmission to an acute care facility.Demonstrated organizational, judgment and decision-making skills.Exceptional leadership abilities – including ability to develop and maintain personal relationships, effective conflict management and team building skills.Demonstrated ability to manage and prioritize a demanding workload and work in a fast-paced, dynamic work environment.Effective time management and prioritization skills.Conflict resolution and strong communication skills.Must be able to work independently and within a team environment.Ability to problem solve and demonstrated critical thinking skills.

Preferred Qualifications:

Masters prepared or in process – in Nursing, Health Administration, Public Health, Leadership or Business.Proficiency in the following computer applications: Connect Care, Microsoft Office Suite, Pathways, RAI Tools, Meditech, and e-People.Experience working as a Charge Nurse, Team Lead, Assistant Head Nurse or equivalent.
