JOB TITLE Youth and Family Counsellor - Supporting Families Clinician

Job Summary:

Within the Supporting Families Program, this position will provide child/youth groups and family centered services where a parent has a mental illness/addiction concern.

Knowledge and experience with family systems, child protection, child development, mental health and addictions. Team work and a family and community centered lens is required.

Scope of practice includes coordinating/facilitating/co-facilitating groups for child/youth and parents, interdisciplinary case conferences, identifying community resources for clients and collaborating with program leadership and partnering agencies.

This position will provide assessment, treatment planning and implementation, case consultation, counselling, service navigation, referral and support services for child/youth and families actively involved in, or being affected by mental health and substance use and other addictive behaviours.

Supporting Families Clinician will liaise as appropriate with staff at other organizations and program leadership to ensure high quality person and family centred care.

Status:

Team / Program Coordination

  • Participates in the development of program specific activities.
  • Receives client feedback, inquiries, and complaints, responds and reports as required.
  • Completes and maintains related records and documentation such as statistics, progress reports, activity participation reports.
  • Performs food coordination and program supply, purchase and preparation.
  • Assists clients in developing realistic ongoing plans for recovery and connects them to appropriate community resources as necessary for follow up or continued treatment.
  • Provides case consultation with program staff and other health professionals to ensure a consistent and integrated case management plan that responds to clients' needs, and file management.
  • Advises Executive Director of any case concerns regarding program progress and safety issues.
  • Facilitating group leader meetings and planning sessions as well as facilitating groups.
  • Establish group and activity schedules for the program.
  • Coordinates involvement of other staff, Practicum students and volunteers.
  • Ensures that proper statistics are collected for program evaluation
  • Safeguarding staff and client privacy and safety and contributing to related policies and procedures.
  • Experience with systems level change management/quality improvement via program planning, implementation, evaluation.

Clinical Care & Support

  • Evaluates the specific nature and extent of a given individual's problems,prioritize these problems with respect to complexity, urgency; and determine the most appropriate service.
  • Accepts referrals, completes intakes and can administer and interpret assessment tools.
  • Responds to crisis situations with referrals to services such as emergency mental health services, detox, hospitals, housing, financial assistance, suicide and violence prevention, and child protection issues.
  • Assists families with system navigation and connection to relevant services.

Individual and Group Clinical Support

  • Plans, prepares and conducts therapeutic and psychoeducational counselling support for those affected by mental health and substance use. This includes individual, group and or family members.
  • Provides counselling and psychoeducation groups.
  • Connects with clients, family members and volunteers to help coordinate programs and services.

Case Management

  • Maintains accurate case files and records regarding client status, progress and use of services.

Case Consultation

  • Participates in case consultation with staff and other health professionals to implement a consistent and integrated case management plan that responds to clients' needs.
  • Provides liaison, education and advocacy to other professional, agencies and communities.

Education and Training

  • Designs, develops and conducts community educational training sessions to enhance knowledge and skills to health professionals as well as substance misusing individuals and their families. Participates in the regular education series provided by the program.
  • Maintains and updates current skills and knowledge by participating in professional, clinical training program, community workshops/conferences and by reviewing current/ relevant literature.
  • Leads in the implementation of education and awareness sessions to enhance the knowledge and skills of individuals and their families.
  • Participates in ongoing quality improvement.
  • Provides support and training for other facilitators.
  • Performs other related duties as assigned.

Qualifications
Requires a Master's degree in Counselling, or Social Work Program or allied or related health discip
