Job Title: Support Worker

We are seeking a highly motivated and compassionate Support Worker to join our team at Chezranti Community Centre Ltd. As a Support Worker, you will play a vital role in providing support and guidance to individuals and families in need.

Key Responsibilities:
  • Assess and address the needs of clients, developing personalized support plans to promote their well-being and independence.
  • Provide education and training on life skills, such as budgeting, cooking, and time management, to empower clients to make informed decisions.
  • Collaborate with community agencies and healthcare providers to ensure comprehensive support for clients.
  • Develop and implement behavior management programs to promote positive change and reduce challenging behaviors.
  • Conduct regular assessments and evaluations to monitor client progress and adjust support plans as needed.
  • Provide crisis intervention and suicide prevention services to clients in need.
  • Supervise and support clients in group homes and halfway houses, ensuring their safety and well-being.
  • Accompany clients to appointments and social outings, providing emotional support and advocacy.
Requirements:
  • 2 years of experience in a related field, such as social work, counseling, or youth work.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and colleagues.
  • Strong problem-solving and crisis management skills, with the ability to remain calm and composed in high-pressure situations.
  • Ability to work independently and as part of a team, with a strong commitment to collaboration and mutual support.
  • Fluency in English, with the ability to communicate effectively with clients and colleagues.
