About the Opportunity

Please note: This role requires strict adherence to the Integrated Chronic Care Services (ICCS) scent-free policy.

ICCS offers comprehensive assessments and care planning for individuals with complex chronic conditions like post-COVID, fibromyalgia, chronic fatigue syndrome, and environmental illness.

The ICCS model focuses on addressing the holistic needs of individuals to manage chronic disease outcomes effectively. The interdisciplinary health care team integrates physical, psychosocial, and environmental needs, emphasizing the person over the condition to enhance functional health and quality of life.

The Occupational Therapist (OT) collaborates with patients, caregivers, and families to optimize self-care, productivity, and leisure. Responsibilities include:

  • Provide individualized OT assessment, consultation, treatment, evaluation, and follow-up.
  • Collaborate on comprehensive care plans for optimal functional health and quality of life.
  • Coordinate care delivery, monitor team-based care plans, and develop group programs for self-management and functional well-being.
  • Manage a busy caseload, prioritize daily issues, and maintain excellent interpersonal and communication skills.
  • Adhere to NSHA policies, professional standards, and contribute to quality improvement initiatives.
  • Support and mentor student learners.
About You

If you have the following, we'd love to hear from you:

  • Graduate of a Bachelor's Degree in Occupational Therapy from a recognized School of Occupational Therapy
  • Registered with the College of Occupational Therapists of Nova Scotia (COTNS)
  • BCLS certification
  • Demonstrated effective communication and interpersonal skills
  • Willingness to adhere strictly to ICCS scent-free policy
  • Skills/experience in motivational interviewing and behavior change theory
  • Competencies in other languages an asset, French preferred

Compensation and Incentives

$33.97-$42.89 Hourly
