Manager, Access and Flow - Permanent Full-Time

Position summary:

The Manager, Access and Flow plays a key role in optimizing the throughput of patients throughout the continuum of care offered at Runnymede Healthcare Centre (RHC) which includes admissions, discharges, and coordination of community and social services. The Manager, Access and Flow is accountable for providing leadership to the Patient Flow and Discharge teams while ensuring efficient management of human and fiscal resources

Responsibilities include:

  • Leads the development, use and updating of evidence-based admission criteria for each program and service to ensure they reflect our programs accurately
  • Models and leads the provision of seamless, prompt, friendly, client focused service to patients, families, clients, staff (internal and external), and the general public in the functions of admission, transfer, and discharge.
  • Works with internal and external stakeholders to optimize the flow of patients, from pre-application to post-discharge, ensuring optimal bed utilization.
  • Manages an effective referral/application process and waiting list system and provides regular reports and updates on these processes to the Director
  • Provides information to and advises the Director, regarding patient flow progress and issues, volumes, metrics, targets and the strategies for improving this flow.
  • Uses expert knowledge of Alternate Level of Care (ALC) while acting as first point of contact in escalation process for ALC avoidance meetings with patients and families in an effort to facilitate a discharge to community.
  • Provides leadership to the patient flow team (Patient Flow and Discharge Coordinator in monitoring lengths of stay for current patients and coordinating safe and timely discharges or program designation changes.
  • Conduct clinical assessment to evaluate appropriateness of referred patients based on established program’s criteria, including patient’s ability to succeed within a rehabilitative care delivery model.
  • Evaluates the application for admission to determine appropriateness based on admission and clinical criteria.
  • Informs the referral source of decision of the application for admission.
  • Establishes the initial contact with the referral source.

The successful candidate will have the following qualifications/experience:

  • Master’s degree in a health care related field, with good standing in the respective college, if applicable, or equivalent experience
  • Knowledge of health care utilization pressures and familiarity with Home and Community Care Support Services is an asset
  • 10 years of experience in a healthcare setting.
  • 5 years experience in leadership capacity, preferably in a similar role
  • Ability to multi-task effectively and to adapt to change in direction to meet timelines and quality of service
