Description

Reporting to the Transitional Housing Supervisor, the Client Service Coordinator act as the first point of contact to program residents. This role is responsible for engaging clients in developing individualized service plans and facilitating access to services and resources to assist with client goals. The Client Service Coordinator works closely with a variety of internal and external stakeholders in support of client, program and organizational objectives.

This is a part-time role working 16 hours per week, 11:00 PM to 7:00 AM Saturday and Sunday. This role is not eligible for a hybrid work arrangement.

WHAT YOU’LL DO

Service Delivery

  • Engage clients using a strengths base approach and utilizing YW Calgary values and principles.
  • Assess client needs and support with the development and implementation of a client-centered service
  • In collaboration with the Clinical Case Manager (CCM), assist clients in creating and applying safety plans.
  • Regular consultation with CCM regarding acuity and level of care needed.
  • Identify services and resources appropriate to the clients’ service plan, support clients with community referrals to various services and resources, attend appointments as needed and advocate on clients’ behalf.
  • Coordinate referrals with mental health, community engagement and housing teams, as

Administration

  • Adhere to sound record keeping and documentation practices, including safe storage of confidential information, timely data entry, and objective case
  • Maintain documentation and records in accordance with program policies and organizational standards.

Professional Development

  • Demonstrate a commitment to continuous learning and improvement; adhere to training requirements, set professional development goals and engage in various learning opportunities.
  • Proactively maintain knowledge of best practices related to issues that impact clients such as homelessness, addiction, domestic violence, and mental health.
  • Attend and engage in staff meetings, supervision meetings, and team building meetings as required.
  • Participate in activities in support of the organizational goals, i.e. Women’s March and fundraising, etc.
  • Build professional, collaborative and effective working relationships with other team members and other internal and external stakeholders and partners.

WHAT YOU’LL BRING

  • Post-secondary education in Human Services with at least two years of related experience, a combination of experience and education may be considered.
  • Current CPR and First Aid Certification, ASIST,
  • Knowledge of issues specific to women experiencing homelessness including poverty, trauma, domestic violence, addictions and mental
  • Experience working with diverse populations (including vulnerable and marginalized populations).
  • Conflict management, de-escalation and resolution
  • Proficient in the use of computers and software including Microsoft Word, Outlook and Excel.
  • Strong written communication skills and ability to complete case notes and other documentation.
  • Excellent communication, relationship building and organizational
  • Commitment to empowering women through a strengths-based
  • Ability to work well under pressure and support overall team needs as
  • Demonstrated ability to work independently as well as part of a team
  • A clear Police Information Check, including a Vulnerable Sector Search, and an Intervention Record Check is required upon commencement of employment.
