This process involves assessment, identification of strengths and needs, goal-setting, treatment planning, and implementation of treatment in a direct, indirect or consultative manner, as indicated, then discharge.This position may be required to work with children and youth with a complex profile of needs to deliver an interdisciplinary approach to care with community partnering agencies in an outreach capacity in the home, community or school.This is just a snapshot of what you would be doing.
