What You Will be Doing:
Program Development & Implementation:
• Develop specific criteria and attributes that would identify residents with mental health and clinical needs
• Research, design, and implement all components of mental health programs
• Provide consultation, assessment, development, implementation and evaluation of individual plans of care
Evaluation and Quality Improvement:
• Create a measurement based program plan
• Design a approach for urgent and developing mental health needs
• Conduct rounds to ensure plans of care are meeting the needs of the residents
Interdisciplinary Collaboration:
• Collaborate with leadership and team members to ensure programs are designed and implantation aligns with Sienna mission, vision and values
Who You Are:
• You are a Registered Nurse, an Advanced Practice Nursing, or a Regulated Health Professional in a relevant field. You are in good standing with your associated regulatory College
• Require 5 years clinical experience involving clients with mental health and addiction issues
• Design experience in developing program using evidence based practices
• Experience in leading change, monitoring and measuring outcomes, and the ability to provide advanced clinical care
• You have excellent interpersonal, communication, negotiation, teaching and computer skills
• You are proficient with MS Office, Outlook, Word and Excel
