Position Summary: The Community Outreach Nurse will play a crucial role in building and nurturing relationships within the community, generating new client leads, and assist with in-home client assessments and care plan creation.
This position requires a dynamic individual with excellent communication skills, a passion for senior care, and the ability to work independently while collaborating with our internal team.
Key Responsibilities:
Community Relationship Building:
- Cultivate and maintain strong relationships with community stakeholders, including physicians, social workers, and senior service organizations.
- Educate the community about the benefits of in-home senior care and the services offered by Senior Helpers.
- Act as a liaison between the company and the community, addressing inquiries and concerns with professionalism and empathy.
Client Lead Generation:
- Develop and implement strategies to identify and attract potential clients for Senior Helpers in-home care services.
- Identify and collaborate with local influence centers i.e.
hospitals, senior living communities, independent and assisted living, memory care, skilled nursing, rehabilitation, respite care, hospice, home health, and physician practices to create referral networks and drive new business. - Attend community events, health fairs, and networking meetings to promote our services and establish connections.
Client Assessments:
- As needed, accompany our Client Service Manager to client homes to conduct thorough assessments and determine their care needs and suitability for in-home services.
- As needed, collaborate with our Client Services Manager to develop individualized care plans based on assessment findings.
- Provide clear and compassionate communication to clients and their families about care options and services.
Administrative and Reporting Duties:
- Maintain accurate and up-to-date records of outreach activities, client interactions, and assessments.
- Prepare regular reports on lead generation activities, community engagement efforts, and client assessment outcomes.
- Assist with the development of marketing materials and strategies to enhance outreach efforts.
Qualifications:
- LPN (Licensed Practical Nurse) or Registered Nurse (RN) license in British Columbia.
- Minimum of 2 years of experience in senior care, home health, or a related field.
- Proven experience in community outreach, sales, or business development preferred.
- Strong interpersonal and communication skills, with the ability to build rapport with diverse populations.
- Exceptional organizational skills and attention to detail.
- Self-starter: Ability to work independently and as part of a team in a fast-paced environment.
