CARE AND BE CARED FOR - THIS IS YOUR HOME
The Nurse Practitioner, NPSTAT position is responsible to support initiatives to reduce transfers from long term care to hospital by providing direct care to long term care residents and supporting long term care homes within a designated geographical area.

Please note that this is an attending position at a designated LTC home.

What will you do:
1. Manages the delivery of coordinated quality care programs in designated long-term care facilities
2. Work within the full scope of practice and will autonomously diagnose, order and interpret diagnostic tests and prescribe pharmaceuticals within the NP legislated scope of practice to avert transfers to emergency departments and hospital admissions
3. Provides patient assessment, diagnosis, treatment, pain and symptom management to clients.
4. Develops collaborative relationships with long term care home staff, Ontario Health atHome Care Coordinators, physicians, hospital staff and others
5. Manages complex and difficult patient issues which cannot be handled in a routine manner
6. Collaborative consultation with physicians to maintain patient centered care
7. Participates and provides effective advice and guidance to Ontario Health atHome staff to help manage difficult and unusual cases
8. Supports Ontario Health atHome initiatives and implementation into practice, including Behavioural Supports Ontario
9. Supports data collection and program review consistent with NPSTAT objectives
10. Designs and implements material and strategic for enhance of clinical competency and capacity of long term care staff to address clinical complexity in the long term care environment
11. Mentor and assist in orientating new NPs and support other clinical programs as directed by NP leadership

What must you have?
• Master’s degree, any relevant field
• Current RN(EC) registration with the College of Nurses of Ontario (with an NP certificate in Primary Health Care or Adult Care)
• Working knowledge of the Ontario Health atHome's role in the long term care sector
• A minimum of 3 years nursing RN experience
• A true passion for making a difference in the lives of our long-term care residents
• Critical thinking skills
• Demonstrated leadership, ability to work autonomously but also as a team
• Collaborative
• Empathic and patient centered

What would give you the edge?
• Gerontology Nurse Certified Canada preferred (GNC)
• Emergency department, mental health/dementia-related behaviours and/or geriatric experience would be an asset
• Strong clinical experience preferred

Hours of Work
Monday to Friday 10am-6pm, subject to change as per the collective agreement (35 hours per week; 70 hours per bi-weekly pay period).

Position Location and Travel
Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
