Your Opportunity:

The Addiction Recovery and Community Health (ARCH) team is a consulting service located at the PLC, specializing in addiction medicine. The ARCH team is multi-disciplinary and comprises Physicians, Registered Nurses, Social Workers, Peer Support Workers, and Pharmacists, providing wrap-around services to patients (s) who present with active substance use concerns. The ARCH team specializes in supporting the primary care team to offer a full spectrum of services through a recovery-orientated care model, including harm reduction, low-threshold services, trauma-informed care, and interventions to improve social determinants of health. The goal of the ARCH team, according to the grant, is to help patients start or progress on their recovery journeys through timely identification of those in need of support, and by offering immediate access to a recovery-oriented system of care (ROSC). Reporting to the manager, you will be an active member of a multidisciplinary addiction medicine consult service primarily supporting acutely ill patients who are experiencing substance use disorders. The role will work with patients who have been consulted by ARCH, in collaboration with the wider team, to provide addiction counselling support, in alignment with the full scope of practice outlined in the provincial job description. The role primarily consists of assessment (identifying the stage of change), developing treatment plans (relapse management planning, harm reduction strategies, skill building) and connecting patients with community resources for treatment (referrals/applications), all in support of patient recovery-oriented care).

Description:

You will develop and implement treatment plans and provide a range of interventions which offer least intrusive and most appropriate services to clients and their families. You will also monitor and evaluate client progress and achievement of recovery goals and adapt treatment as needed. As an Addiction Counsellor III, you will depend on your excellent relationship building skills with clients, families, government staff and colleagues.
