Description

Outreach Workers work in a team to provide client services for homeless and other marginalized individuals, to network with other community service and health agencies and maintaining a client record database.

Note: This is a unionized position.

KEY RESPONSIBILITIES:

The incumbent is responsible for the following range of job duties characteristic, or typical of the job class:

Takes responsibility for routine intake processes and decisions, following standard procedures Coordinates intake with the shelter staff; attends case management meetings to report on new intakes Records intake process and decisions utilizing BC Housing Case Planning data base (HIFIS) Attempt to seek out clients needing support in the community and connecting them with appropriate programs at the Centre of Hope Record all methods of communication with clients daily in case files Advocate on behalf of clients where appropriate Develops case histories for all outreach clients. Liaises with referral sources and other external agencies as needed (e.g. welfare, police, parole, urgent situations) Works with government and community funders to ensure funding is in place Conducts comprehensive program orientation for all clients/residents Fosters positive relations with community social services agencies/resources Performs other duties as assigned

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

Education/Certifications:

The successful job applicant will have completed up to two (2) years of Community College.

NOTE: An alternative level of education and experience may be acceptable.

The successful applicant will hold/have:

Certificate or Diploma in Human Sciences/Addictions Criminal Police Investigation Check (CPIC) Appropriate immunization Measures Current Certification in First Aid/CPR Valid Driver’s License

Experience:

Minimum of 3 years of prior related experience including, frontline work in a social services setting/experience in a related field.

Other Requirements:

Excellent writing and oral skills Solid administrative skills Training in Crisis and Skills that focuses on the prevention of homelessness. Understanding of the needs of victims of absolute homelessness Ability to work with minimum supervision

The job incumbent may be exposed to the risk of minor to moderate injury or illnesses due to:

Physical attacks by others (i.e. attack by a potential client) Exposure to infectious diseases Sharp objects Other dangerous situations

The incumbent must take the following precautionary measures (in addition to health and safety rules):

Universal Precautions Current Certification in First Aid/CPR Appropriate immunization and vaccinations Armatus Online Abuse Prevention Training
