Reporting to the Manager of Allied Health, the successful candidate will be responsible for the assessing and implementing evidence-based nutrition interventions in an acute care environment. Collaborating within an interdisciplinary care team, the successful candidate will employ evidence-based interventions according to the practice standards set by the College of Dietitians of Ontario and internal policies and procedures. The successful applicant may be required to work in a variety of settings, including inpatient surgical, medical, intensive care, stroke, geriatric rehab, high-intensity rehab, and transitional care or outpatient clinic areas.

 DUTIES:

Screen and identify patients requiring intervention in assigned patient areas using established nutrition care standards. Respond to and prioritize appropriate high risk nutrition referrals received from physicians and inter-professional team members, families and patients. Utilize scientific evidence, physical assessment, medication/nutrient interactions, biochemistry, medical nutrition history and chart review to asses the patients’ nutritional needs. Develop, order, coordinate and evaluate/monitor the implementation of high risk nutritional care plans. Educate patients, families and inter-disciplinary team members. Accurately document all patient nutrition care appropriately in the patients’ medical charts. Participate in the development of hospital policies and procedures specific to directing patient nutritional care. Provide continuing education to inter-professional team members promoting a leadership role as a primary resource for accurate, evidence-based nutrition information. Liaise with support/community groups and institutions as required for the purpose of transfer of patient care to ensure seamless continuum of patient-centered care. Function as a professional role model demonstrating good interpersonal skills and a commitment to self-growth and development. Maintain current knowledge of best practices and benchmarks for the delivery of services, appropriate research, literature, accreditation requirements, policy and legislative requirements and implementing improvements/changes as appropriate. Act as a preceptor/mentor to dietetic interns and other students. Other duties as assigned.

QUALIFICATIONS:

Registration with the College of Dietitians of Ontario in good standing. Experience in enteral and parenteral nutrition. Experience working with medical directives Ability to work effectively and efficiently as a team member in a fast paced environment in stressful situations. Customer service, interpersonal and telephone skills. Computer literacy in a Microsoft environment, flexibility to work within different computer program environments. Ability to maintain successful working relationships with the interdisciplinary team to achieve positive patient outcomes. Ability to organize time effectively to perform the duties of the position. Ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner. Human relations skills including empathy, sensitivity, understanding of human and organizational behavior, motivational techniques and counseling skills. Ability to read, write and communicate to perform the duties of the position. Translating and assimilating scientific, medical and nutritional information into practical individualized treatment plans. Ability to identify nutritional disease states such as malnutrition, iron deficiency anemia, zinc deficiency, etc. through assessment of patient’s diet, skin, physical traits, lab work, medical history, medication and disease knowledge etc. and prescribe nutritional therapy to correct the problem. Develop new and highly specialized methods to treat/manage complex nutritional needs of patients who have numerous co-morbidities.
