About the facility: LLRIBHS promotes health in our communities for the wellbeing of all members. We provide accessible, high quality health services and programs. Together with others, we work to build capacity and educate as we address both acute needs and the determinants of health.
Duties:
Reach out to community members to access the enhanced service coordination; Enhance awareness of existing services and supports for children; Build relationships and collaborate with those providing existing community-based services; Identify children with unmet needs to facilitate early invention and timely access to services and resources; Enhance opportunities for the success of at-risk clients and families; Locate resources; identify options and available supports Facilitate applications to government/community agencies Help families navigate the health and social systems in a timely manner; Help with referrals for assessments to appropriate professionals; Build case management capacity through the development of policies, procedures and protocols related to screening and assessment, service planning and monitoring/evaluation of services plans and staff training Provide a collaborative, client driven system to guide children and their families through a comprehensive array of services; Support a child and family centered approach to coordinating and connection servicesRequirements:
Minimum two year certificate in related field; bachelor’s degree preferred (A combination of education and relevant experience may be considered); 3 years of experience working with First Nations Health, Social and/or Education sectors; and Training and/or experience with assessment/screening tools and case management; Excellent Verbal & Communication Skills; Must possess a valid class 5 driver’s license; Ability to speak/understand Cree is an asset; Understanding of First Nations communities and their challenges and barriers; Current Criminal Record Check, Vulnerable Sector check and Driver’s abstract required upon hire;
