POSITION SUMMARY:

Physiotherapist: Mackenzie Health Outpatient Stroke Program

We are currently seeking a Physiotherapist to work at Mackenzie Health Hospital in Stroke Outpatient Clinic located in Vaughan.The Physiotherapist will provide patient management through all stages of assessment, treatment, consultation and follow-up services to patients.The successful candidate will be familiar working in an outpatient clinic setting.

As the successful candidate, you are a graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists of Ontario.You demonstrate excellent interpersonal, organizational and communication skills, both written and verbal, along with a high level of initiative and adaptability to rapidly changing demands.

RESPONSIBILITIES:

As a Physiotherapist (PT) you will be conducting assessments & treatment plans As a PT you will be formulating goals and recommendations Planning and implementing patient centered, goal-directed treatment programs & working with Therapy Assistants Assist in the promotion of physiotherapy services to external referral sources Participating actively in the planning, implementation and evaluation of the program

REQUIREMENTS:

·Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with a high-performance interdisciplinary rehabilitation team ·Graduate of an accredited program in Physiotherapy and are a member in good standing of the College of Physiotherapists Ontario ·Minimum 2 years clinical experience in Stroke/Neurological patient populations is required ·Experience in assessment and generating treatment plans for patients with stroke/neurological disorders ·Knowledge of stroke best practices ·Experience with Chronic disease management is a good asset ·Sound knowledge of community resources is an asset ·NDT training is an asset ·Effective interpersonal, organizational and communication skills ·Strong customer service focus and excellent problem-solving skills required ·A positive work and attendance record is essential ·Ability to adapt to new computer applications ·CPR + AED certificate ·Valid G license required ·Criminal background check, Vulnerable sector check and Immunization record (current).
