North Simcoe Family Health Team Social Worker - Mental Health Organization Name FTE Type 1.0 FTE Contract Address 619 Prospect Blvd., Suite 3Midland ON L4R 0G3Canada January 22, 2026 Job Description The North Simcoe Family Health Team (NSFHT) is seeking a full time, 12-month contract position. Job Title: Social Worker – Mental Health Salary range: $36.94 - $39.21 per hour Reports to: Clinical Director The NSFHT is a multi-disciplinary team located in Midland Ontario. We provide primary care, education, and counselling services to patients in the North Simcoe Community. We offer a friendly, collaborative, and professional environment. Position Summary As an integral member of the NSFHT, the Social Worker delivers a wide range of mental health services. These services may include management of mental health services, counselling, education, navigation, case management and advocacy. The social worker will mainly service individual adults and groups. Responsibilities and Job Expectations Provide assessment and treatment for patients referred by their health care practitioner or self-referred. Provide social work intervention as part of an interdisciplinary team and support programs across the Family Health Team (FHT). Assist patients and their families in navigating the health care and/or social service system, which may include case management responsibilities. Provide groups and/or educational sessions in response to specific need identified by the FHT. Participate in health promotion activities such as walking groups or health fairs. Maintain clear, concise, and accurate documentation using an electronic medical records system. Actively participate in ongoing program evaluation and quality improvement. Provide education to staff and community about mental health and mental illness. Participate in committee work within the NSFHT and with partnering agencies as needed. Advocate on behalf of a patient or group of patients as needed. Note: The above is intended to describe the general nature of the work being performed by the incumbent of this job. QUALIFICATIONS AND SKILLS Master of Social Work (MSW) preferred; Bachelor of Social Work (BSW) with experience will be considered. Current registration with the Ontario College of Social Workers and Social Service Workers. Must be in good standing with regulatory body. Valid professional liability insurance (we can guide you through this process if you currently do not have liability insurance). Skills/Abilities/Knowledge and Experience Experience in a community based or clinical health service organization. Excellent clinical skills and experience with assessment, treatment, and management of mental health problems commonly seen in primary care. Experience facilitating group programs. Case management/system navigation skills and a willingness to research community resources. Ability to function effectively and efficiently both independently and as a member of the team. Excellent verbal and written communication skills. Excellent time management and organizational skills. Knowledge of electronic medical records (Accuro) an asset. Work Schedule This is a full-time, 12-month contract position with a standard 37.5-hour work week, Monday to Friday.
