As a Primary Care - Nurse Practitioner (Status: Full-Time, Start date: Immediately; Location: North Bay) , you provide primary care to Ontario Health atHome patients who typically do not have a primary health care provider (physician or nurse practitioner) and who are medically frail, medically complex, with numerous chronic illnesses, or have difficulty accessing health care outside of their home. The NP works autonomously within the scope of practice to diagnose, treat, medically manage and monitor patients, and consults with a collaborating physician when outside the scope of practice to manage patients independently, as defined by the College of Nurses of Ontario.

What will you do?
Assess patients' needs and current medical status by completing a comprehensive health assessment to develop a health care plan.- Diagnose diseases, disorders, and conditions and inform patients, their families, caregivers, substitute decision maker (SDM), Care Coordinators and/or Health Practitioners.
- Manage patients with complex chronic illnesses and monitor their treatment response.
- Collaborate and consult with other health professionals and make appropriate referrals to family physicians, specialists, and other health professionals.
- Write prescriptions to treat patients when applicable.
- Create and maintain written records of assessment, intervention, and any contact with patients, their family, caregivers, SDMs, or other health professionals.
- Educate and assist patients and their family, caregivers, or SDMs in health promotion and disease prevention.
- Assess and recognize population health trends, plan, and implement strategies for population prevention and education.
- Balance patient needs and choices with available resources, ensuring patients' values, and preferences are respected.
- Maintain accurate clinical records and documents patient assessment findings, goals, treatment plans, and interventions.
What must you have?
- Nurse Practitioner/Registered Nurse in the Extended Class in good standing in accordance with the College of Nurses of Ontario (Nurse Practitioner);
- Relevant nursing experience working in a community/health care environment;
- Knowledge of home health care and other community resources within Ontario Health atHome;
- Comprehensive knowledge of medical management of the elderly population, chronic diseases, and/or complex care needs of the at risk population;
- Experience in patient, family, and community education;
- Knowledge and experience in implementing evidence-based clinical best practice guidelines;
- Demonstrate critical analysis and assessment skills, problem-solving, and interviewing skills;
- Practical knowledge and understanding of relevant legislation (e.g. regarding the provision of health care services, privacy, health and safety, etc.);
- Comprehensive knowledge of the standards of practice and professional guidelines set forth by the College of Nurses of Ontario, RNAO, NPAO, and other relevant medical professional associations;
- Ability to prioritize professional duties, manage multiple patients, and efficiently organize workload;
- Strong interpersonal and collaboration skills to work with diverse patient groups, Care Coordinators, family members/caregivers/SDMs, health care professionals, community organizations and service providers;
- Effective listening, observation, and facilitation skills;
- Ability to communicate information effectively through a variety of means including reports, letters, meetings, and presentations;
- Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel);
- Completion of controlled substances education which has been approved by the College of Nurses of Ontario;
- Must have valid driver's license and access to a vehicle;
- Valid Emergency First Aid Certification;
- Oral and written proficiency in English and French is essential.
