Housing First Intensive Case Manager - Reaching Home Division Good Shepherd Hamilton & Toronto invites applications for the position of Housing First Intensive Case Manager. HOURS: 37.5 hours per week. Regular shifts Monday–Friday 9:00 am–5:00 pm, one late shift 11:00 am–7:00 pm, and a occasional Saturday day shift every ~6 weeks. Some flexibility may be required. Learn more at How You Will Make An Impact Provide trauma‑informed intensive case management to youth, women and families experiencing chronic homelessness. Work with service users in the community to secure permanent housing and provide ongoing supports for up to 24 months. Partner with agencies to coordinate assets and services in Hamilton to meet individualized housing and support needs. Work in the office, overflow hotels, shelters, and community settings. Accompany users to appointments, apartment viewings, and conduct monthly home visits. Assist users in a hands‑on capacity. Apply strong time‑management skills in a high‑energy environment. Work independently and as part of an interdisciplinary team. Follow Good Shepherd health and safety rules and Occupational Health and Safety Act provisions. What You’ll Bring Post‑secondary education in Social Service or a related field. Minimum 1 year of related experience or an equivalent combination of education, training, and experience. Knowledge of issues affecting youth, women, and families, including homelessness, violence, poverty, mental illness, and substance use. Commitment to person‑centered, trauma‑informed, harm‑reduction, and Housing First frameworks. Excellent interpersonal, conflict‑resolution, communication, time‑management, and organization skills. Ability to collaborate within an interdisciplinary team. CPR certification (or willingness to be certified). Valid G‑license. Fluency in a second language (asset).
