Are you the one we’re looking for?


 


We are seeking a dynamic, highly motivated Intensive Case Manager who demonstrates our values of compassion, accountability, responsiveness, and empowerment to our clients, families, and each other.


 


Reporting to the Manager of Halton Housing Services, this position will assist in providing intake supports and carrying out day-to-day administrative services.  Wesley Halton Housing offers services to the homeless in Halton Region, supporting homeless families and individuals in a transitional program on-site.  Services are based on a client-centered, residential harm reduction, housing first approach to service delivery. 


 


This position is a unionized, full-time, temporary position. The contract is until July 1, 2026. The rate of pay is $24.38.


 


Key Responsibilities: 


 



  1. Works with assigned families and individuals in a housing program that works towards a housing plan. This includes maximizing income, filing taxes, obtaining an ID, and connecting with community support. 
  2. Responds to calls and referrals from community agencies in accordance with the case load.
  3. Places individuals and families into permanent leased units, securing appropriate housing based on individual needs.
  4. Assesses and screens participants’/families’ needs for services to support their transition to stable housing and optimize their capacity to sustain housing using an acuity tool and other resources.
  5. Completes Social & Community Housing applications (HATCH) to submit to the Region, including all required documents for approval, OR ensures existing application remains active.
