Outreach Worker for Homeless Prevention

In this role, you will identify clients at risk of homelessness and develop personalized intervention plans. Use your strong communication skills to provide emotional support and aid in problem-solving as clients navigate housing challenges. Regular participation in case management meetings ensures that you remain aligned with best practices and client progress.
Key Responsibilities: • Make connections with clients experiencing homelessness • Assess client needs and create tailored intervention plans • Provide supporting services to enhance life and social skills • Liaise with community service providers and landlords on behalf of clients • Maintain comprehensive client records and progress reports
Requirements: • Degree in Social Services or related field • Minimum of two years relevant experience • Knowledge of the Housing First model and recovery principles • Strong planning and collaboration skills • Ability to manage stress and maintain professional boundaries
