Key Responsibilities
• Provide comprehensive primary health care services to families
• Evaluate patient health and implement care plans
• Maintain accurate health records and ensure confidentiality
• Engage in community health promotion activities
• Participate in peer review and quality improvement initiatives
Requirements:
• Master’s prepared Nurse Practitioner with relevant certification
• 3-5 years of practical experience in primary care
• Registration with the College of Nurses of Ontario
• Strong communication and conflict resolution skills
• Access to a vehicle and valid driver's license Enhance community health through your expertise at SCHC.
