Family Physician for Diverse Community Health

Contribute to community health as a Family Physician at Pinecrest-Queensway Community Health Centre. This part-time role focuses on providing holistic primary care to diverse and underserved populations. As a Family Physician, you will collaborate within a multi-disciplinary team to offer comprehensive primary care services in the PQCHC area. The role includes diagnosing and managing a range of medical conditions while proactively addressing health maintenance and education. You will also conduct health assessments and coordinate referrals to ensure effective patient pathways. Key Responsibilities:

• Deliver primary medical care following CPSO protocols
• Treat and manage both acute and chronic health issues
• Work with a team of health professionals for integrated care
• Complete health assessments and develop comprehensive plans
• Educate clients about chronic disease management and prevention

• Doctor of Medicine (MD) and CPSO licensure
• Minimum of two years in family medicine
• Experience with health equity for priority populations
• Proficient with PS Suite EMR and Microsoft Office
• Strong communication and interpersonal skills
